Brightshores Health System is announcing its new support care coordination program for residents across Grey and Bruce.
The MyCareSteps program is a collaboration with Serefin Health, and is available at no cost to all residents across the region, and complements care provided by physicians and care teams.
MyCareSteps provides care coordination between patients and nurse care coordinators, who provide virtual support.
It also means that the nurses can help patients and their families understand their care plans, help them manage their conditions between appointments, and connect health and community services.
The program began on April 1st, by accepting an initial cohort of patients with complex or ongoing care needs, with a particular need during transitions between care settings.
Residents of Grey and Bruce Counties can get access to MyCareSteps through a referral from their primary care provider, a hospital-to home referral after discharge from hospital, or by self-referral through mycaresteps.ca/greybruce.
